Capstone Health: Top 2025 National Community Health Quality Awardee

Capstone Health receives recognition as one of the Top 2025 Community Health Center Quality and Public Health Champion for Diabetes Health and Heart Health in the nation.

The U.S. Department of Health and Human Services (HHS), through the Health Resources and Services Administration (HRSA), announced the Top 10% of health centers receiving Community Health Quality Recognition (CHQR) badges for their achievements in improving health outcomes and providing high-quality care for patients in rural and underserved communities.

The CHQR badge program honors the health centers across the country that provide excellent quality of care to patients by HRSA clinical quality measures standards and clinical quality performance. This includes a variety of clinical areas, such as behavioral health, maternal health, diabetes health, heart health, cancer screening, and HIV prevention and care, as well as overall quality performance.

Capstone Health believes quality, complete health care is for everyone. In 2024, Capstone Health served its communities with 27,611 medical visits, 1,281 behavioral health visits, 13,754 outreach assistance visits and filled over 72,000 prescriptions.

Capstone Health provides access to affordable primary, preventative, and wellness services distinguished by mission, vision, integrity, and respect in Walker County, Winston County, and surrounding areas.

These efforts reflect a broader national impact, with approximately 1,400 HRSA-supported health centers operating nearly 15,000 service sites across the country. Together, these centers cared for 32.4 million patients in 2024—the largest number served in the Health Center Program’s 60-year history.
